The Problem of Acne

Acne and Comedones are common problems that plague our skin. Manifesting as white or blackheads to large inflammatory lesions such as nodulocystic acne that if left untreated, can leave behind permanent scars. Acne can bring about long-lasting damage to one’s psyche and self-confidence.

There are two main types of acne we experience – inflammatory and non-inflammatory acne. Inflammatory acne occurs when the pores in the skin are deeply clogged with bacteria (Propionibacterium Acnes), oil and dead skin which eventually leads to intense swelling and redness. Non-inflammatory acne, on the other hand, occurs closer to the surface of the skin and may not have bacteria overgrowth.

What is Photothermal Therapy?

Photothermal Therapy is the use of electromagnetic radiation to heat particle molecules to allow them to deal localised heat damage to the targeted area without dealing collateral damage to the surrounding tissues. This is used to eliminate excess sebaceous glands to reduce sebum production. It also helps to cleanse the remaining sebaceous glands of built-up dirt, oil and bacterial organisms.

At Cambridge Medical, Gold Nanoparticles are used to infiltrate the pilosebaceous unit and stimulated by laser energy to control the activity of sebaceous glands. Gold is also known for its incredible ability to absorb the light energy emitted by lasers.


How is Gold PTT treatment carried out?

Step 1 – Deep Cleanse With AquaJet Facial

The surface of the skin is thoroughly cleansed to remove any dirt and oil accumulated throughout the day.

Though the surface of the skin may be clean, dirt, oil and dead skin can still accumulate beneath the layers. The AquaJet Facial aims to cleanse deeper into the first layer of the skin – the epidermis by using a Vortex jet and suction method.

It uses a tool to deliver an active ingredient (Lactic Acid) to remove dead skin cells and balance oil production. It is then moved around the face while making use of salicylic acid to loosen any dirt from the pores effectively clearing out blackheads. Finally, a potent triple solution consisting of Portulaca Extract, Hyaluronic Acid and nourishing agents are massaged into the skin for long term skin health.

Step 2 – Apply Gold Nanoparticles

A layer of Gold Nanoparticles in suspension will be applied topically onto the skin.

These gold-coated silica nanoparticles will then, in the later steps, be absorbed into the skin with ultrasonic waves, followed by irradiation of PICO laser to induce focal thermolysis of the sebaceous glands without any collateral damage of the skin.

Step 3 – Penetration of 24K Gold using LDM® Tech

After the application of the 24K Gold Nanoparticles, LDM® tech is used to massage it into the pores. LDM®, also known as local dynamic massage, makes use of rapid ultrasonic frequencies to penetrate these nanoparticles deep into the pores.

On top of this, LDM® provides the added benefit of controlling enzymes that lead to metabolic tissue conditions such as acne, activates heat shock proteins that enable the skin to combat stressful conditions such as exposure to extreme temperature and increases the moisture content in the skin to improve overall skin tone.

Step 4 – Activation of Gold PTT using PICO laser

PICO lasers are the fastest lasers (450 picoseconds) skin lasers available. This ensures that there is no heat damage to the surrounding tissues while still allowing for the lasers to accurately target the gold particles. They absorb the light energy and vibrate fast enough to heat up and destroy the targeted sebaceous glands.

Finally, to wrap up the entire treatment, the Omega Light Low-Level Laser Therapy is used to stimulate the body’s natural healing response. The Omega Light uses yellow and infrared wavelengths to stimulate healing and reduce inflammation.


The difference between PTT and PDT

PDT, or Photodynamic Therapy, was once considered the gold standard for such acne treatments. It entails the use of LED light in combination with topical creams or a sensitizer called aminolevulinic acid (ALA). However, it had a multitude of unwanted side effects such as redness, swelling, crusting, and even the hyperpigmentation flare.

PTT, on the other hand, is the extension of PDT whereby the sensitiser is replaced by a photosensitiser (the gold particle) which is excited to heat the specific sebaceous glands. This helps to effectively treat acne with minimal downtime and discomfort.

Is the procedure painful?

Clients often only feel a pinprick sensation during the laser treatments. Other than some redness, there is essentially no downtime and you would be well on your way to going about your day to day activities after the procedure. How long before I can see results?

Each treatment is 45 minutes long. It is recommended that clients come in for treatment once every 2 – 3 weeks for 5 to 10 sessions.
